What Is the NCLEX in 2026?

The NCLEX (National Council Licensure Examination) is the standardized exam required for nursing licensure in the United States. You must pass the NCLEX-RN (for registered nurses) or NCLEX-PN (for practical nurses) before you can work as a licensed nurse. As of 2023, the NCLEX transitioned to the Next Generation NCLEX (NGN) format — a major update that changed both the question types and the scoring model.

In 2026, all NCLEX candidates take the NGN version. The exam uses Computerized Adaptive Testing (CAT) — meaning it adjusts question difficulty based on your answers. The minimum number of questions is 85; the maximum is 150. Most candidates finish between 85 and 130 questions.

Next Generation NCLEX (NGN) — New Question Types Explained

📝 Extended Multiple Response

Select ALL options that apply from a list of 5-10 choices. Partial credit available. More nuanced than traditional SATA.

🔗 Extended Drag and Drop

Match items, order sequences, or categorize nursing actions. Tests clinical reasoning and prioritization.

📊 Cloze (Drop-Down)

Fill in the blank by selecting from drop-down menus within a sentence or clinical note. Tests precise clinical knowledge.

📊 Matrix/Grid

Check multiple boxes across a table — e.g., which interventions are indicated or contraindicated for a patient.

📈 Trend

Review a series of patient data over time and identify changes in condition. Tests clinical judgment over a patient timeline.

📋 Case Studies (6-item)

New in NGN: 3 standalone case studies with 6 questions each. Follow one patient through assessment, diagnosis, planning, intervention, and evaluation.

NCLEX 2026 Passing Standard

The NCLEX uses a logit-based passing standard — not a percentage score. The current passing standard for NCLEX-RN is 0.00 logits (updated with NGN). For NCLEX-PN it is -0.18 logits. You won’t see your logit score — you’ll see Pass or Fail (or the Quick Results service shows “Likely Pass/Likely Fail” 48 hours after testing).

The CAT algorithm continues giving questions until it is statistically confident (95% confidence interval) that your ability is either above or below the passing standard. If the exam ends at 85 questions, it means the computer was confident enough to make a determination early — in either direction.

How to Study for NCLEX 2026 — NGN-Specific Strategies

1

Shift Your Focus to Clinical Judgment, Not Memorization

The NGN NCLEX tests the NCSBN Clinical Judgment Measurement Model (NCJMM) — a 6-step framework: Recognize Cues, Analyze Cues, Prioritize Hypotheses, Generate Solutions, Take Actions, Evaluate Outcomes. Every question tests at least one of these steps. Study the framework explicitly and practice applying it to patient scenarios, not just memorizing content.

2

Practice NGN-Specific Question Formats Daily

Traditional NCLEX prep resources (Saunders, Davis, older Kaplan) are not sufficient for NGN. You need resources that include Extended Multiple Response, Matrix/Grid, Cloze, and Case Study questions. UWorld NGN edition, ATI’s NGN resources, and NCSBN’s own practice exam are the current gold standards. Use these exclusively in the final 4-6 weeks.

3

Master the 6-Item Case Studies

Case studies are worth more than individual questions because they test the full clinical judgment cycle. Practice reading EHR-style documentation, identifying relevant vs. irrelevant cues, and making clinical decisions across all 6 steps. Aim to complete at least 10 full case studies under timed conditions before your exam.

4

Use ATI Comprehensive Predictor and HESI Exit as Benchmarks

Your ATI Predictor score is the single best predictor of NCLEX success available before you test. A Level 2 predicts approximately 95%+ NCLEX pass probability at most programs. If you haven’t hit Level 2 on the ATI Predictor yet, you need to address that before scheduling NCLEX. ExamplifyHelp.com guarantees Level 2 or Level 3 — call (310) 990-6314.

5

Do 75-100 NCLEX-Style Questions Per Day for 8 Weeks

Volume and quality of practice questions is the strongest predictor of NCLEX success. Target 75-100 questions daily for 8 weeks before your exam. Review every wrong answer in detail — understand the clinical reasoning, not just the correct answer. Students who complete 3,000+ practice questions before NCLEX pass at significantly higher rates.

6

Schedule Your NCLEX Within 3 Months of Graduation

NCLEX pass rates decline significantly with time since graduation. Students who test within 3 months of completing nursing school pass at the highest rates. Don’t delay your application or testing date. Make sure your ATI Predictor and HESI Exit scores are where they need to be before you apply — call (310) 990-6314 if you need those guaranteed first.

Need to Pass ATI or HESI Before You Can Apply for NCLEX?

Most nursing programs require you to pass the ATI Comprehensive Predictor (Level 2+) or HESI Exit (850+) before they allow you to graduate and apply for NCLEX. If you’re blocked on either of those exams, ExamplifyHelp.com can guarantee your score.

We take your ATI Predictor, HESI Exit, or any Examplify-proctored nursing exam remotely with a written score guarantee before you pay. Pre-exam connection test included. Same-day available.

NCLEX 2026 — Frequently Asked Questions

How many questions is the NCLEX in 2026?
The NCLEX-RN in 2026 uses Next Generation NCLEX (NGN) format with Computerized Adaptive Testing. The minimum is 85 questions and the maximum is 150. Most candidates see between 85-130 questions. The exam ends when the computer reaches 95% statistical confidence that your ability is above or below the passing standard — not after a fixed number of questions.
What is the NCLEX passing rate in 2026?
The overall NCLEX-RN first-attempt pass rate for US-educated candidates is approximately 82-86% as of the most recent NCSBN data. International candidates have lower pass rates. The passing standard was updated with the NGN transition. Your ATI Predictor Level 2+ score predicts approximately 95%+ NCLEX pass probability.
What is the Next Generation NCLEX (NGN)?
The NGN is the updated NCLEX format launched in 2023. It introduced new question types (Extended Multiple Response, Matrix/Grid, Cloze, Trend, Case Studies) designed to test clinical judgment using the NCSBN Clinical Judgment Measurement Model. As of 2026, all NCLEX candidates take the NGN version. Older study materials from before 2023 do not fully prepare you for NGN question formats.
How long do I have to pass the NCLEX after graduating nursing school?
There is no federal time limit for taking the NCLEX after graduation, but most state BONs require you to apply within a certain period of receiving your authorization to test (ATT). ATT is typically valid for 90 days. You must also meet your state’s application requirements. NCLEX pass rates decline with time since graduation — testing within 3 months of graduating gives you the best odds.
Does ExamplifyHelp.com help with NCLEX prep?
ExamplifyHelp.com specializes in the nursing exams that come BEFORE the NCLEX — ATI Comprehensive Predictor (Level 2/3 guaranteed), HESI Exit (850+ guaranteed), TEAS 7, HESI A2, and all Examplify-proctored nursing coursework exams. We can help you clear every hurdle between you and your NCLEX application. Call (310) 990-6314 for a free quote.
What happens if I fail the NCLEX?
If you fail the NCLEX, you can retest after a 45-day waiting period. Most states allow unlimited retakes, though some have caps. Each retake requires a new application and fees. The most important step after a failed NCLEX is identifying your weak content areas from your candidate performance report and targeting those specifically in your retake prep.
